Insurance

While I do not take insurance, I can provide you with a superbill which you may be able to submit to your insurance provider as an expense toward your deductible. Your insurance plan may include partial or full reimbursement for private pay mental health services. This policy varies by insurer, so please ask your insurance company what benefits are for mental health care and out-of-network providers.

Additionally, counseling qualifies as an expense you may pay for with an HSA/MSA.

Life happens and you may occasionally find that, due to unforeseen circumstances, you need to cancel an appointment. Please notify us as soon as possible so that we may offer  the time to someone who may be waiting for an appointment.

We ask that  you cancel or reschedule your appointment at least 24 hours in advance of  your scheduled start time to avoid being charged appointment fees.

Our full cancellation policy is as follows:

  • Clients are allowed (1) unexpected cancellation without the client being charged for the appointment.
  • If a second (2) cancellation occurs, the client will be charged one-half the fee for  that appointment.
  • In the event of a third (3) or any subsequent cancellations, the client will be charged for the full amount of the appointment.

Forms

As a new client, you will be required to complete some online forms prior to your first therapy session including a (1) New client intake form and (2) Informed Consent/Consent to Treat.

Additional forms such as an authorization to release information may be required if you would like your Counselor to coordinate care with another  provider (for example, your psychiatrist, primary care physician, etc.). Your counselor will discuss any additional important forms and practice procedures with you at your first session.
